Director of Patient Services
Greater Augusta Area Full-Time Leadership

Senior leadership role overseeing operational and clinical coordination of home health services. You'll provide strategic direction, staff leadership, and operational oversight to the Care Coordination team.

Key Responsibilities

  • Lead, supervise, and mentor the Care Coordination team including intake staff and scheduling personnel
  • Oversee patient intake, referral management, scheduling, authorization, and care coordination
  • Develop and evaluate departmental workflows, policies, and procedures
  • Monitor KPIs including referral conversion rates, start-of-care timeliness, and patient satisfaction
  • Champion compassionate, patient-centered service delivery across the department

Qualifications

  • Senior-level experience in home health operations or patient services leadership
  • Strong knowledge of Medicare/Medicaid home health regulations
  • Proven ability to lead teams and drive operational performance

Care Coordinator
Greater Augusta Area Full-Time Patient Services

Central point of contact for patients, families, referral sources, and clinical staff. You'll ensure home healthcare services are initiated, scheduled, and delivered seamlessly from referral through completion of care.

Key Responsibilities

  • Receive and process referrals from hospitals, physicians, and skilled nursing facilities
  • Verify patient eligibility including insurance benefits, Medicare/Medicaid coverage, and prior authorizations
  • Coordinate start-of-care scheduling and assign appropriate clinical staff
  • Serve as primary liaison between patients, caregivers, referral sources, and the clinical team
  • Manage visit schedules for nurses, therapists, and home health aides

Qualifications

  • Experience in home health intake, care coordination, or medical office scheduling
  • Familiarity with insurance verification and authorization processes
  • Strong organizational and communication skills

Physical Therapist (PT)
Greater Augusta Area Full-Time Therapy

Evaluate patients in their home environment, design individualized treatment plans, and deliver evidence-based physical therapy interventions that restore function, reduce pain, and improve mobility.

Key Responsibilities

  • Perform comprehensive PT evaluations assessing strength, ROM, balance, gait, and functional mobility
  • Develop goal-oriented plans of care with patients, families, and the interdisciplinary team
  • Provide skilled services including therapeutic exercise, neuromuscular re-education, balance/gait training, and manual therapy
  • Complete OASIS assessments as required by Medicare home health guidelines
  • Design and instruct patients in safe home exercise programs

Qualifications

  • Current Georgia PT license (or eligible)
  • Home health experience preferred; new graduates welcome

Physical Therapy Assistant (PTA)
Greater Augusta Area Full-Time Therapy

Under the supervision of a licensed Physical Therapist, carry out individualized treatment plans and support patients in restoring mobility, strength, and functional independence.

Speech-Language Pathologist (SLP)
Greater Augusta Area Full-Time Therapy

Evaluate and treat patients with communication disorders, cognitive-linguistic impairments, and swallowing dysfunction. Deliver individualized, evidence-based speech therapy in the patient's home.

Key Responsibilities

  • Conduct comprehensive SLP evaluations assessing communication, cognition, voice, fluency, and swallowing
  • Administer and interpret standardized and non-standardized assessment tools
  • Provide skilled services targeting language, speech intelligibility, cognitive-communication deficits, and dysphagia
  • Complete OASIS assessments per Medicare home health guidelines

Qualifications

  • Current Georgia SLP license (or eligible)
  • ASHA Certificate of Clinical Competence (CCC-SLP)
